We facilitate access to existing community resources, including healthcare, housing support, and social services. Thus creating necessary pathways to essential services that can make a significant impact on individuals' lives. These collaborations ensure that we are aligned with community efforts and can advocate for our clients effectively and avoid duplication of services.
*Complex Care Coordination*
Our outreach workers assist individuals with complex cases by navigating the system alongside them. We working diligently to address challenges such as wait lists for housing, services, shelter accessibility, and identifying the appropriate welfare departments or programs for assistance. This may include follow-up support services after program discharge.
*Employment and Education Services*
Connection to local job placement programs, that include but are not limited to recovery friendly employment agencies, Voc. rehab, NHEP, as well as provide volunteer and training opportunities to strengthen the skill set for employment.
*Basic Needs*
Access to food pantries and meal programs.
Laundry/Shower Services

Support for individuals involved in the criminal justice system, including community corrections, drug court, and mental health court.
Welfare/Police After-Hour Support for local authorities for urgent client needs.

Arranging transport thru doorway or insurance for recovery services and medical appointments.

Support services from individuals with lived experience.

*Legal Assistance*
While we are not qualified to provide legal advice, our team can connect clients with New Hampshire Legal Assistance (NHLA) and the Public defender's office
- Promote compliance by assisting in tracking community service hours, court dates, and probation/parole requirements.
